By submitting this application, I agree to the conditions in effect at the time of this application and any subsequent changes by the Georgia Professional Standards Commission and/or the College of Education. I also understand that the Teacher Education Admissions Officer reserves the right to review my progress for the purpose of determining my continued acceptance in the Teacher Education Program. If I am denied admission, I understand that I must meet any new requirements at the time I reapply.

I agree that, if accepted, I will abide by all the rules, procedures, and policies of the College, now and hereafter adopted, as set forth in the Mercer Catalog, the Code of Ethics for Educators, and the Honor System. I understand that the falsification of information in any admissions document is grounds for denial of admission and/or may result in dismissal from Mercer University.

For Certification-Only Students: If I accept a position as the teacher of record, it will be in the field in which I am being prepared, or I will meet with my advisor and/or the Office of Field Placement staff prior to accepting the position to determine the impact that might have on my certification and degree completion.

Except for minor traffic and parking violations, have you been arrested, formally accused, cited, fined, posted bail, or ordered to perform community service for the violation of any law?(Required)

Provide dates of any incidents, arrests, convictions, suspensions, etc., in your explanation. Because of the high ethical standards to which teachers are held, failure to disclose an act or event such as the ones described above is often significant and leads to serious consequences. Failure to provide truthful or complete answers and/or failure to inform the admissions officer of any changes to answers may results in revocation or admission or in disciplinary action by the College of Education, and/or detail of certification by the Georgia Professional Standards Commission. In accordance with standard requirements for admission, you must disclose each instance even though the charges may have been dismissed or you were acquitted or adjudication was withheld or a conviction was reversed, set aside, or vacated or the record sealed or expunged, regardless of whether you have been told you need not disclose any such instance. Charges of driving or boating under the influence of alcohol or drugs must be disclosed. Felony convictions must be disclosed. Such convictions will result in a denial of Certification by the Georgia Professional Standards Commission.
